Check-in comment Periodic autofocus now includes a plan-start autofocus. GEM:752

Here's the rub: #autofocus is not a "plan start" directive, it is a "per target" directive". Thus, the #autofocus at the beginning of your plan is just an autofocus for the first target. The next night, the first target has been completed and the next one in line (the 4th or whatever) does not have an #autofocus, so...

I am going to change the behavior of #afinterval to include a plan-start autofocus. I know it will probably be a surprise for a few people but it makes more sense. The presence of an initial #autofocus (as currently recommended in my docs) would be harmless. The surprise will be for people who explicitly don't want the plan-start AF. I'll live with the backlash. I think most people want to start out with an autofocus when they have #afinterval set.
